Feels like a bit of an unnatural playing styles but maybe it's me not being used to it. It however does force your hand in a slightly awkward angle that I could see causing some wrist strain overtime.
It seems less of button layout issue and more of your playstyle, I personally suggest switching your right thumb out for your right index for a more natural hand position and see how that plays out for you, not sure how easy that would be for you so maybe expect a slight skill drop but shouldn't be anything too severe I hope.
If you however don't want to go down that route you could alternatively perhaps have a keybind where the buttons are more vertical to avoid that awkward angle like instead of right and left arrow keys you could use something like:
Thumb : /
Middle finger : ] or ' and maybe even \

My main causes for concern, are exactly wrist pain, and t pain.
The reason for this layout was that I liked playing Chicken Invaders with the same controls.
For the right hand, I have tried both in the past few days:
index on left arrow, and middle finger down arrow,
0, and - on numpad.
Results were the same, more than 1 star down each position, with bad accuracy.
Meanwhile reading skills degraded.
Maybe, the best option is playing both old and new layouts for now...
