open the file with winRAR, then take the folder in the .rar file and drag it into your skin folder, wich is located the same place you installed osu. If you have osu running then restart it and you're done

Open the Skins folder inside your osu! installation folder and then extract the folder that is inside the .rar archive to that folder. After that you should be able to see the skin inside your skin list in osu!.

It doesn't need to be an .osk for you to be able to use it. Just extract the skin folder from the .rar file and put it in your Skins folder, located in your osu! installation folder (usually at C:\s\[YOURNAME]\AppData\Local\osu!).
